For your first appointment in our offices please arrive 20 minutes before your appointment to obtain needed health information. Once you are accepted, you can fill out some of these forms before your first visit. It is encouraged for you to fill out and turn in the form to obtain your medical records.
Please bring the following:
- Photo I.D.
- Co-pay payment, if applies to your insurance
- Insurance card
- List of medication, dosages, and frequency. If medication is controlled, it is needed to bring the bottle.

We are happy to provide your own personal electronic health record access.
You can sign up for this at your first visit. This highly secure, HIPPA compliant, and convenient communication tool allows you to do the following:
- Request appointment
- Request prescription request
- Review Lab results
- Email the staff
- Review medical record summary
- Review immunization records
- Review balances

The electronic healthcare record is now a requirement by the government. We have chosen a system that is highly secure, fast, and convenient. Appointment time
We ask you to contact us in advance if unable to make the appointment. We try to be fair and have appointment available. No shows decrease that possibility and increase the time available for a follow up, and may refills.
